North Carolina Insurance Commissioner Mike Causey announced today the arrest of Jonathan Michael Vlasak, 35, of 4388 Sandy Pine Court, Battleboro. Vlasak, a licensed insurance agent, was charged with 74 total counts, including 25 counts of forgery, 25 counts of identity theft, 22 counts of obtaining property by false pretense and two counts of attempting to obtain property by false pretense. All these charges are felonies.
According to special agents with the N.C. Department of Insurance’s Criminal Investigations Division, Vlasak assumed the identities of 16 different victims and forged life insurance applications under their names to obtain $8,256.07 from Occidental Life Insurance Co. of North Carolina and an additional $3,751.48 from Americo Life Insurance Co. of North Carolina on 22 separate occasions.
According to the arrest warrants, the offenses occurred between Aug. 23, 2022, and Oct. 25, 2022.
Vlasak was arrested on June 4, released under $100,000 secured bond plus a $10,000 unsecured bond and ordered to appear in Nash County District Court on June 6.
“This was an extraordinary case and I thank our agents for staying with the investigation and uncovering this scheme,” Commissioner Causey said. “Consumers need to be extra vigilant to protect sensitive information such as this from getting into the hands of the wrong person. The department is committed to helping protect consumers by pursuing and prosecuting these types of major crimes in North Carolina.”
